Hemodynamics in congenital mitral stenosis.

چکیده
The hemodynamic findings in five patients with congenital mitral stenosis, four of whom resided at mild altitude (5,200 to 6,500 feet) are presented. Pulmonary hypertension was present in all patients. Reactivity of tbe pulmonary vascular bpd was noted in the four patients residing at altitude. The administration of tolazoline and/or 100 percent 0 2 to three patients resulted in an average drop in mean pulmonary artery pressure (PAP) of 20 mm Hg (range 13 to 33) and in TPR of 790 dynes/M2 (range 350 to 1,200). In one patient catheterized 36 hours after being transferred to sea level mean PAP declined from 85 to 40 mm Hg and TPR decreased from 1,590 to 730 dynes/M? These studies have shown the presence of markedly elevated TPR in patients with congenital mitral stenosis who reside at 5,200 feet. The pronounced reactivity of the pulmonary vascular bed with drugs and/or increase in oxygen tension suggests that reactive vasoconstriction, due m part to mild hypoxia, is an important factor in maintaining the high TPR in these patients.

Double orifice mitral valve (DOMV) is an uncommon anomaly that was first described by Greenfield. Since then more than 200 cases have been reported. In this anomaly, mitral valve has a single fibrous annulus which opens with two openings into the left ventricle. We report a case of DOMV accompanied with severe stenosis in a 22-year old female who had complaints of moderate dyspnea. In transth...
متن کاملAnomalous pulmonary venous return with intact atrial septum: diagnosis and pathophysiology.
Twenty-one patients with partial anomalous pulmonary venous drainage with intact atrial septum have been studied. These include 13 patients not previously reported from our laboratories and eight patients with complete hemodynamics reported by others. Methods for identification of this abnormality and for identification of an intact atrial septum are described, including differential indicator ...
متن کاملImportance of accurate diagnosis using real-time three-dimensional echocardiography in the surgical treatment of congenital intramitral ring in infants.
Congenital mitral ring is a rare type of congenital mitral stenosis. There are two types of mitral ring: the intramitral ring and the supramitral ring. Intramitral rings are often associated with complex valve pathology, and therefore have a worse prognosis than supramitral rings, in which the mitral apparatus is usually normal. The role of echocardiography in the evaluation of the mitral valve...
متن کاملTetralogy of Fallot with rheumatic mitral stenosis: A case report
INTRODUCTION Rheumatic and congenital heart diseases account for the majority of hospital admissions for cardiac patients in India. Tetralogy of Fallot is the most common congenital heart disease with survival to adulthood. Infective endocarditis accounts for 4% of admissions to a specialized unit for adult patients with a congenital heart lesion. This report is unique in that a severe stenotic...
متن کاملPacman Heart as a Congenital Cardiac Defect Associated with Flail Mitral Valve and a Partial form of Shone\'s Complex; A Case Report
Partial muscular inter-ventricular septal defect (VSD) or Pacman heart is a rare congenital or occasionally acquired anomaly. Concurrent Pacman heart and Shone's complex are extremely rare and have never been reported until now. We described a 37-year-old male patient with congenital Pacman heart, flail mitral valve (FMV), and a history of multiple congenital anomalies, including subvalvular ao...
